APPLICATION REQUIREMENTS 2010-2011

All students need to submit a completed and signed application form, birth certificate, and immunization record. In addition to these items:

• Preschool students need to be 3 years old by September 30, 2010 and fully toilet trained  

• Kindergarten students need to be 5 years old by September 30, 2010 and submit $50 assessment fee  

• First – Fourth Grade students must submit a $50 testing fee and a completed and signed Transcript Request Form (from St. John’s)  

• Fifth – Eighth Grade students must submit a $50 testing fee, a completed and signed Transcript Request Form, and complete a student questionnaire (from St. John’s)  

All applications will be kept for the school year for which the application was made. The application process begins anew each and every school year.
